MEDICAL DIRECTOR:
- Co-certifies the terminal prognosis with attending physician
- Leads the interdisciplinary team in development of care plan
- Provides consultation to other physicians regarding hospice care
SOCIAL WORKER:
- Assesses emotional, social, spiritual and financial needs of patient and family
- Develops plan of care to meet identified needs
- Provides direct counseling or refer patient and family to appropriate community agencies
- Provides bereavement support
SPIRITUAL COUNSELOR:
- Assesses spiritual needs of patient and family
- Develops care plan to meet identified needs
- Assists with memorial preparation
- Provides consultation to community clergy
- Provides spiritual counseling
BEREAVEMENT AND GRIEF SERVICES:
Grief is painful. Ultimately, there is no way around it, only through it. Our hospice bereavement services are open to the community and help people make this transition and adjust to their new
life. Services are provided by hospice staff and volunteers trained in grief and loss. If you are concerned about your or someone else’s reactions to grief, our bereavement counselors are always
here to help.
- Develop care plan to meet survivor needs for up to 13 months after patient’s death
- Develop memorial services
- Maintain verbal and written communication with family and friends
- Provide grief counseling and healing resources
VOLUNTEER SERVICES:
- Provides companionship and support to patient and family
- Provides needed non-medical services and respite time for caregivers and family
- Provides emotional support to family and friends at time of death and during bereavement
HOSPICE AIDE:
- Provides direct personal care to patient
- Provides comfort measures
- Reports identified needs to RN case managers
NURSING CARE:
- Assesses patient and family needs
- Develops care plan to meet identified needs
